How to register

The doctors welcome new patients who live within our practice area.

To register with the practice you need to fill in a GMS1 form which can be printed from our website or picked up from reception. Please then bring this in with Photo ID ie current passport or driving licence, along with a utility bill or other proof of address and your National Insurance card/NHS number.

Please then book an appointment with our healthcare assistant for a new patient check, which involves checking your medical history and medication.

Medical treatment is available from the date of registration. Please contact reception for further information.

However, if you need medication on repeat prescription you will need an appointment with a GP first.

Should you have a preference for a particular doctor/doctor gender, please make these views to reception when you hand in your registration forms.

Newly registered patients

All new patients are invited to have a general health check with the practice nurse. This includes a height, weight and blood pressure check. It also allows us to summarise your medical history while awaiting the arrival of your notes from your previous doctor. Please try to bring details of your past illnesses, vaccinations and smear history (where applicable), as well as a sample of urine for testing.You can collect a specimen pot for this from the reception desk.
